Two follow-ups for the approval refactor:
- InputReducer STEER input mode hardcodes APPROVE; needs the future
  approve/reject overlay UX to let the user pick.
- GlobalStreamHandler derives scopeSessionId from requestId as a
  placeholder; needs a real requestId → sessionId lookup once exposed.
